Viewing and Downloading Tax Forms – User Guide

This guide provides step-by-step instructions on how to view and download your tax forms from Zeal Realty’s Tax Form Portal. If this is your first time using the portal, you will need to provide electronic consent before accessing your documents. Follow the steps below to securely retrieve your tax form.

1. Type your email address and click "Confirm"

2. Go to your email to view your One Time Password, type it on the verification form and click "Verify"

If is your first time using this portal you will need to provide your electronic consent and signature before accessing the form.

3. Type the last 4 of your SSN, EIN or TIN provided to Zeal and click "Submit"

4. Click on Form 1099

5. From This page you can View and Download your form
